Free Whitening For Life

Dr. Sundstrom and his team know how important it is for you to have a healthy and bright smile. We pride ourselves on making sure that our patients have a smile that they can be proud of. Whitening for Life is an exciting new program that is offered to our new and existing patients. We would like to whiten your teeth and keep them white for life at no extra charge to you.

Here is how it works:

New Patients:

1. First, we would like to see you for an initial exam, cleaning, and x-rays. Call our office at 423-894-0650 to set up you first appointment.
2. Once your teeth are clean and healthy, you may invest in your initial whitening procedure. We will provide you with custom fitted whitening trays, for $150.00 (a $200.00 savings- promotion good until December 31, 2013), to cover the cost of materials.
3. Then one complimentary whitening syringe will be awarded to you at your scheduled cleaning two times a year.

Existing Patients:

1. If you have already had whitening trays made in our office, you are automatically enrolled in our new exciting Whitening for Life program after signing our Whitening for Life agreement. Then, as you come in 2 times per year for your dental checkups, you will receive a whitening syringe at no cost to you!
2. If you have not already had whitening done in our office and you would like to join our Whitening For Life program you can enroll for the one-time low enrollment fee of $150.00 (a $200.00 savings - promotion good until December 31, 2013 ) for the custom fitted professional whitening trays. Then as you come in 2 times yearly for your regular dental checkups, you will receive a whitening syringe at no cost to you.

To be eligible for our Whitening for Life program you need to meet the following requirements:

• Keep your regularly scheduled dental appointments. Your long-term oral health is as important to us as it is you. Our patients have found that regular checkups greatly reduce dental emergencies. This is why we are happy to provide this extra bonus to our patients who are committed to their oral health.
• Provide at least 48 hours notice if you need to postpone or change appointment. In order to provide exceptional services, like Whitening for Life, to all of our patients, we ask that you give us the courtesy of advanced notice for all schedule changes. Reconnection fees will apply
• Must keep your mouth free of gum disease and cavities.
• All accounts must be current (no outstanding bills).
• Must be 18 years of age.

That’s it!!!! To remain in the program, all we ask is for you to maintain good home care, come in for regularly scheduled preventative care that is recommended by Dr. Sundstrom, and avoid missing appointments.
